dc.description.abstractBackground: To reduce the MMR, it is done by ensuring that every mother is able to access quality maternal health services, such as health services for pregnant women, delivery assistance by trained health workers in health care facilities, postnatal care for mothers and babies, special care and referrals if needed. complications occur, and family planning services including family planning after childbirth. One of the efforts to reduce maternal, infant and child mortality is to carry out continuous care (continuity of care). Objective: To provide continuous midwifery care from pregnancy, childbirth, postpartum, newborn and family planning. Method : The method and documentation used is SOAP management. Result : Mrs. K 33 years old, GIVPIIIA, HPHT 30-06-2020, TTP 09-04-2021 had 3 visits in the third trimester. There was itching in the genital area at 32-34 weeks of gestation, can be overcome by encouraging the mother to maintain personal hygiene, change underwear if it is wet or damp and dry the genitals with a clean and absorbent cloth. In the normal delivery process, Mrs. K had a grade I perineal rupture, sutured and no problems were found in the treatment of the perineal wound. The baby was born spontaneously, weight 3600 gr, body length 50 cm, male gender, Apgar score 9/10. The lactation process went smoothly and Mrs. K wanted to be a 3 month injection family planning acceptor Conclusion: The care provided starting from pregnancy to becoming a family planning acceptor is in accordance with the standards of care and the authority of the midwife.en_US
