Abstract: | Background: Maternal mortality and morbidity is still a serious health problem in developing countries. Based on the district/city health profile report in North Sumatra province, the number of maternal deaths reported in 2018 was 185 people with the distribution of maternal deaths 38 people, maternal deaths 79 55 people and maternal mortality during the puerperium. Efforts to reduce maternal, infant and child mortality, one of which is to implement continuous care (Continuity of Care). Objective: To provide continuous midwifery care from pregnancy, childbirth, postpartum, newborn and family planning. Method : Case study method and documentation with SOAP management. Result : Mrs. F 32 years old, G2P1A0, HPHT 04-06-2021, TTP 11-03-2021. 4 visits during pregnancy. During labor, NY.F's amniotic fluid was cloudy, Mrs. F suffered a grade II perineal rupture, sutured and no problems were found in perineal wound care. The baby was born spontaneously, BB 3300 g, PB 50 cm, male sex, Apgar score 8/10. The lactation process went smoothly and Mrs. F became an acceptor for 3 months injectable family planning. Conclusion: The care provided starting from pregnancy to becoming a family planning acceptor is in accordance with the standards of care and the authority of the midwife |
