Abstract

Background : Factors causing perineal rupture consist of maternal factors such as parity, precipitous parturition, prolonged labor, maternal age Efforts to improve the quality of maternal and child health services, one of which is to carry out continuous care or continuity of care. Purpose: To provide midwifery care to Mrs. P Age 37 years with continuity of care starting from pregnancy, maternity, postpartum, newborns and family planning in accordance with midwifery care standards. Methods: Midwifery care with 7 varney rare and documentation with SOAP management. Results: After a midwifery examination, Mrs. P found the problem of frequent urination, but it was resolved by notifying the physiology of the third trimester of pregnancy. The birth process of the baby was born spontaneously with a weight of 3400 grams, body length 50 cm, Apgar score 8/10, with female sex. During the second stage of care, Mrs. P suffered a grade II rupture, but the problem can be resolved by proper perineal wound care. The care for BBL was given as needed and there were no danger signs or complications in the baby. At the last visit during the postpartum period, she was informed about the use of contraception, the mother decided to become an injectable family planning acceptor which was injected once every 3 months. Conclusion: The care provided starting from pregnancy to becoming a Devo Provera family planning acceptor is in accordance with the standards of care and the authority of the midwife.
