Abstract

Background: Health services for pregnant women must meet the minimum frequency in each trimester. Assessment of the implementation of health services for pregnant women can be done by looking at the coverage of K1 and K4. Objective: Applying midwifery care to Mrs. N Age 18 years with continuity of care in accordance with midwifery care standards with a management approach Methods:Continuous midwifery care and documentation with SOAP management. Result : Mrs. N is 18 years old, GI P0 A0 gestational age 31-32 weeks, HPHT 28- 06-2020, TTP 05-04-2021. ANC performed with pregnancy in good condition. The baby was born spontaneously and immediately cried, with male sex, weight 3000 g, body length 48 cm, Apgar score 8/10, no congenital abnormalities. During delivery, the mother experienced a 2-degree perineal rupture. In the implementation of postpartum care, there were no problems or normal conditions, and no complications were found. There were no problems with the new born baby. At the end of the postpartum visit, family planning counseling was carried out and the mother chose to become an acceptor of 3-month injections (depoprovera). Conclusion: In this case, Mrs. N is 18 years old and is in good condition. Mothers are encouraged to consume nutritious foods, Fe tablets and become family planning acceptors
