Abstract

Background: Midwifery care prioritizes continuity of care, it is very important for women to get services from the same professional or from a small team of professionals, because after being given continuous care the client will be more open in expressing complaints, and feel calm there. who accompanies the examination and monitors the condition of the client and the fetus, gets more knowledge, so that all risks that endanger the mother and the fetus can be addressed immediately. Purpose: To provide midwifery care to Mrs. R Age 29 years with continuity of care starting from pregnant women, childbirth, childbirth, newborns and family planning acceptors in accordance with the standards of midwifery care and midwifery management. Methods : Continuous midwifery care by documenting SOAP in accordance with midwifery care standards. Result: Continuous midwifery care for Mrs. R 29 years old, GIIPIA0. The process of giving birth to a baby born spontaneously with a weight of 3400 grams, PB 51cm, LK 33cm, LD 34cm, A / S: 8/10 with the male gender. During the implementation of the first stage of labor, it was found that the mother had prolonged labor with a labor duration of 15 hours . In the implementation of the second stage of childbirth care, Mrs. R experienced grade II rupture but immediately given action, namely hecting grade II using the basteful technique. The care given to Newborn baby according to normal Newborn baby needs and there were no signs of danger or complications in the baby. At the last visit during the postpartum period, the mother was informed about the use of contraception and the mother decided to become an injectable family planning acceptor. Conclusion: In this case Mrs. R 29 years old with prolonged labor and grade II perineal rupture. The services provided are in accordance with the standards of care and authority of the midwife.
