Abstract

Background :Maternal mortality rate is one indicator to see the success of maternal health efforts. In general, there was a decrease in maternal mortality during the period 1991-2015 from 390 per 100.00 live birth. Objective : Improve the knowledge and ability to equip ourselves with science and skills to give The Midwifery Continuity of care in pregnancy,maternity, postnatal, newborn baby and family planning use the midwifery management. Method :Continuity of midwifery care management with subject Object Assessment Planning management. Result :After midwifery care was given to Mrs.A during the delivery process, the baby was born spontaneously with weight: 3400 grams, body length of 50 cm, appearance pulse grimace activity respiration score 8/10, female. It was found that the second degree perineal rupture in the delivery process was found in Mrs. A . Care for BBL was given as needed and no danger signs or complications were found in the baby . At the last visit during the puerperium, he was informed about the use of contraception. Conclusion : This case Mrs. A is 31 years old at the time of delivery there was a grade II perineal rupture
