ASUHAN KEBIDANAN PADA NY.Y MASA HAMIL, BERSALIN, NIFAS, BAYI BARU LAHIR, DAN KELUARGA BERENCANA DI PRAKTIK MANDIRI BIDAN H.P KOTA PEMATANGSIANTAR

Abstract

Background : In 2018-2019 the Maternal Mortality Rate (MMR) decreased from 4.226 to 4.221 maternal deaths in Indonesia. The highest number of maternal deaths were bleeding 1.280 cases, hypertension in pregnancy 1.066 cases and infection 207 cases. The number of maternal deaths in Pematangsiantar City in 2018 in East Siantar District was found to be 2 deaths caused by bleeding and hypertension in pregnancy. Early marriage is a marriage that is carried out at the age of teenagers, the minimum age of marriage for men and women is 19 years. Based on the Indonesian Demographic and Health Survey (IDHS), the number of women aged 15-19 years increasing in rural areas has increased to 36%. The percentage of early marriage in urban areas is 26% and the gestational age of adolescents, namely from the age of 15-19 years, is 1,97%. Objective : To provide midwifery care to Mrs.Y Age 23 with continuity of care starting from pregnant women, maternity, postpartum, newborns and family planning acceptors in accordance with midwifery care standards and midwifery management. Method : Continuous midwifery care and documentation with SOAP management. Result : Mrs.Y is 23 years old G3P2A0 in the third trimester of pregnancy, the first contact is 32-34 weeks of gestation. At the time of delivery there were no tears and no problems with the baby, the newborn was treated normally. Monitoring is carried out during the postpartum period to assess the normality of the process experienced until becoming acceptor injection for a 3-month family planning acceptor. Conclusion : The care provided starting from pregnancy to becoming a family planning acceptor injection for a 3-month is in accordance with the standards of care and the authority of the midwife
