Abstract: Back ground of problems:The implementation of health efforts in various areas is basically aimed at achieving the goal of health development to bring about the highest degree of public health through accessibility,affordability,and quality of health care and thus able to anticipate changes in development of problems and challenges to health development themselves. Purpose: To provide care in obstetrics at Mrs. D age 26 years old with continuity of care started from pregnant women, maternity, childbirth, and newborn and acceptor family planning in accordance with the standards care of midwifery and midwifery management. Method: Continuity midwifery care and documentation by SOAP management. Result: Care in midwifery in a sustainable manner of Ms. D pregnancy with mild anemia does not lead to complications in pregnancy and childbirth. The baby was born spontaneously with the weight of 3700 gr, the body length of 50 cm, apgar score 9/10 with woman gender. Care on a newborn baby given according to the needs of baby’s. In a recent visit of the childbirth, has been informed about the use of contraception. The patient decided to become acceptor family planning injection 1 month. Conclusion: In the case of Mrs. D 26 years old with mild anemia, it is found no gap between theory and practice. The patient is recommended to consume nutritious foods, Fe tablet 1x1, and be acceptor family planning
