Abstract

Background : Continuous care needs to be done as a measure of success or as an important indicator of the health of mothers and babies. Maternal health should begin when a woman is preparing for pregnancy, during pregnancy, childbirth, postpartum and breastfeeding, using family planning contraception until old age. Purpose : To provide a comprehensive care service or continuity of care in performing its role sake of the health of mothers and children so as to reduce Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). Method : The method used is ongoing midwifery care and documentation with SOAP management. The results : Mrs. Yanti 23 years, GI P A0 gestational age 32-33 weeks, HPHT 06-22-2020, TTP 03-29-2021. The implementation of the ANC was not found to be a problem. During childbirth, the baby was born healthy without complications, cried immediately, the sex was male, the baby's weight was 3,700 grams, and IMD was performed immediately, successfully at 60 minutes. Grade II perineal rupture occurred and could be treated according to the existing procedures. The postpartum period of Mrs. Y did not got any problem, the lactation process went smoothly and the baby was breastfed. Conclusion :The care provided from the time of pregnancy to becoming a family planning acceptor, it runs normally. So the effort to improve health quality for pregnant women is to carry out comprehensive care or continuity of care so that it can reduce the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR).
