Abstract

Background: The health status of mothers and children in the family can be seen from the Maternal Mortality Rate and Infant Mortality Rate. This is one of the targets for achieving and assessing the degree of welfare of the people in a country itself. Purpose: The purpose of writing is to improve the health status of mothers and children by implementing continuity of care for pregnant women, childbirth, postpartum, newborns, and family planning. Method: The method used is ongoing midwifery care and documentation with SOAP management. Result: Mrs. Z, 21 years old, GII PI A0, HPHT: 07-06-2020 TTP: 14-03- 2021, 3 visits, complained of frequent BAK. This can be overcome by advising the mother to change clothes whenever they are wet or damp to avoid itching in the vaginal area. In the normal delivery process, Mrs. Z experienced second degree perineal rupture, suturing was performed and there were no problems in the treatment of the perineal wound. Babies born spontaneously with weight 3400 gr, body length 50 cm, head circumference 34 cm, chest size 33 cm with male sex, in the care of newborns no complications were found. The baby received breast milk, the umbilical cord puput on the day 5. During the postpartum period, Mrs. Z did not experience any complaints, the lactation process went smoothly and the baby was willing to breastfeed. Ms. Z's family planning care was given information about selected effective contraceptives such as AKDR , AKBK, injections and pills. Conclusion: The application of the midwifery care mindset carried out at Mrs. Z with continuity of care is expected to be a benchmark and guide in providing midwifery services.
