ASUHAN KEBIDANAN PADA NY.D MASA HAMIL, BERSALIN, NIFAS, BAYI BARU LAHIR DAN KELUARGA BERENCANA DI PRAKTEK BIDAN MANDIRI R.M KOTA PEMATANGSIANTAR

Abstract

Background: Mothers and children are family members who need to get priority in organizing health efforts, because mothers and children are vulnerable group. This is related to the phases of pregnancy, childbirth and puerperiumfor mother and the phase of growth and development for children. Objective: to provide midwifery care for 36-year-old Mrs. D in contiunity of care start from pregnancy childbirth, postpartum, newborns and family planning in accordance with the standards of obstetric care and obstetric management. Methods: Continuous midwifery care and documentation with SOAP management. Results: Continual midwifery care to Mrs. D pregnancy had no problems. The process of childbirth went spontaneously with weight 2900 gr, body length 50 cm, apgar score 8/10 with male gender. During the second stage of care, Mrs.D experienced a degree I rupture and performed a second degree hecting with the jeluda technique. Newborn care was given as needed and there were no signs of danger or complications found in the baby. At the last postpartum visit was informed about the use of contraception, the mother decided to become a 3-month injectable family planning acceptor. Conclusion: In this case Mrs. 33 years old with perineal rupture, there was a gap between theory and practice. The baby was in good condition, there were no complications whatsoever and have received vit K and HB0 immunizations. The mother have received 3 months of injection of birth control
