Abstract: | ackground:TheHealthProfileofNorthSumatraProvincein2018was 185peoplewithadistributionof38maternaldeaths,79maternaldeaths and55postpartum maternaldeaths.Theabilityofacountry'shealth servicesisdeterminedbytheratioofhighandlow maternalmortality ratesandperinatalmortalityrates.AKIisoneindicatorformaternal success.AKIisthematernalmortalityrateduringpregnancy,childbirth andpostpartum causedbypregnancy,childbirth,andpostpartum orits managementbutnotduetoothercausessuchasaccidentsorfallsin every100,000KH Purpose:Thepurposeofthispaperistoimprovethehealthstatusof mothersandchildrenbyimplementingcontinuityofcareforpregnant women,maternity,postpartum,newborns,andfamilyplanning. Method :The method used with secondary and primary data and documentationwithSOAPmanagement. Result:Mrs.D24yearsold,GIIIPIAI30-32weeksgestation,HPHT22-06 -2020,TTP 29-03-2021.ANC carriedoutbythemotherinahealthy conditionandthefetusisingoodandactiveconditionaswellasthe administrationofFemedication,3visits,thebabywasbornspontaneously withweight3200gr,bodylength50cm,headcircumference33cm,chest size32cm,malegender,Apgarscore9/10.Mrs.Ddidnotexperienceany complaints,thelactationprocesswentsmoothlyandthebabywantedto breastfeed.Mrs.Dbecameanacceptorfor1monthinjections. Conclusion:TheapplicationofmidwiferycarecarriedoutonMrs.Dwith continuityofcareisexpectedtobeabenchmarkandguideinproviding midwiferyservices. |
