Abstract: | Background : The number of maternal deaths in Indonesia 2019 was 4,221 maternal deaths, however the number of MMR in Indonesia is still high at 305 per 1000 live births. According to the Pematangsiantar City Health Office, in 2018 IMR there were 3 per 1,000 live births, the biggest cause of infant death was heart defects. Purpose : To provide care in obstetrics for Mrs. L age 27 years old with continuity of care started from pregnancy, maternity, childbirth, and newborn until become acceptor family planning in accordance with the standards care of midwifery and midwifery management. Method : Continuity midwifery care and documentation by SOAP management. The Result : The baby was born naturaly with the weight of 3200 gr, the length of 50 cm, apgar score 9/10 with female gender. In the implementation of the care II, Mrs. L got rupture but the problems can be resolved by doing hecting with baste technique. Care on a newborn baby given according to the needs of baby. In a recent visit of the childbirth, has been informed about the use of contraception. The patient decided to become acceptor family planning injection once for three months. Conclusion : In the case of Mrs. L 27 years old with rupture perineum, it is found no gap between theory and practice. The patient is recommended to consume nutritious foods, Fe tablet, and become acceptor family planning |
