dc.description.abstractBackground : Health story of mother and child in family can be seen from maternal mortality and infant mortality. This make one of achievement and valuation target level public welfare in a country it self. Purpose : Writing purpose is to increase health level mother and child by applying continuity of care to pregnant women, maternity, childbirth, and newborn and family planing. Method : The method is continuity midwifery care and documenting was using Subjective Objective Assessment Plan management. Result : Mrs. F age 23 years old, GIIPIA0, HPHT 10-06-2020, had 3 visits, On the process normal childbirth, Mrs. F had a grade II perineal rupture. sewing and there was no problem in treating perineum wound. Newborn baby weight : 3100 gram, length 50 cm, lingual 33 cm, bust 32 cm, gender female, Apgar Score 8/10. Baby was get breast feeding, the umbilical cord broke on day 4. puerperium period Mrs.F feeling well. The lactation process get well and baby want to suckle. Mrs. F being an acceptor to a family planing a three month injection. Conclusion : Obstetrics service performed on Mrs. F gestation birth until it becomes breeders with continuity of care midwiery applying thought patterns according to the standard of nursing care and midwife authority.en_US
