Abstract

Background:MMRisoneindicatortoachievethesuccessofmaternal healthefforts.TheresultsoftheindonesianDemographicandHealth Survey(IDHS)in2018AKNat15per1,000livebirths,IMR24per1,000 livebirths,andAKABA32per1,000livebirths.TheToodlerMortality Ratehasreachedthe2030SustainableDevelopmentTarget(TPB) whichis25/1,000livebirthsanditishopedthatAKNcanalsoreachthe targetof12/1,000livebirths. Objective:ToapplymidwiferycarewithcontinuityofcaretoMrs.Ais 31yearsoldstartingfrom pregnantwomen,givingbirth,postpartum, newborns,andfamilyplanningaccordingtpstandartofcareusing SOAPdocumentationwithamidwiferymanagementapproach. Metode:ContinuousmidwiferycareanddocumentationwithSOAP management Results:InthecaseofMrs.A,aged31,duringpregnancy,therewere noproblems.Thebirthprocessofthebabywasbornspontaneously withaweightof3500grams,bodylength48cm,Apgarscore8/10, withfamalegender.Theimplementationofthesecondstageofcarefor Mrs.A sufferedagradeIIruptureandperformedgradeIIhectingwith thebastingtechnique.Thecarefornewbornbabywasgivenasneeded andtherewerenodangersignsorcomlicationsinthebaby.Atthelast visitduringthepostpartumperiod,shewainformedabouttheuseof contraceptions,themotherdecidedtobecomeafamilyplanning acceptorafter6weekspostpartum Conclusion:Inthiscase,Mrs.A31yearsoldwithrupturedperenium in normaldelivery,thereisnogapbetweentheoryandpractice.Thebaby isingoodcondition,therearenocomplications,andhasreceived vitaminKandHb0immunizations.Mothersareencouragedto consumenutriousfoodsthataimtoprovideadequatebreastmilkfor thebaby
