dc.description.abstract | Background: documentation deaths occurred during childbirth and 1 death occurred during the postpartum period. Maternal mortality rate is calculated based on the number of deaths due to pregnancy, childbirth and postpartum. Information about the high maternal mortality rate will be useful for reproductive health improvement programs, especially services during pregnancy so that pregnancy is safe and free from high risks. The program to increase the number of births is realized by the factor of health workers, the available referral system to handle pregnancy complications, and the family and husband being alert in welcoming the birth. The prevalence of maternal mortality in Pematangsiantar in 2018 reached 5 cases, 4 Objective: This study aims to improve the health status of mothers and children through continuity of care for pregnant women, maternity, postpartum, newborns, and family planning services. Methods: Continuous midwifery care and using SOAP management. Results : Mrs. D, 27, GIIPIA0, the first day of the last menstruation was 02-07- 2021, given 7 visits, the delivery process was normal, had a second degree perineal rupture, hecting was done and no problems were found in the care of the perineal wound, the baby girl born spontaneously weighing 3000 g, length 50 cm, head circumference 33 cm, circumference 32 cm the baby is breastfed, the umbilical cord is lagging on day 9, during the puerperium Mrs. D did not experience any complaints, the lactation process went smoothly and the baby wanted to breastfeed, No complications were found in the care of newborns, and Mrs. D became an acceptor of the 3-month injection method as a means of pregnancy control. Conclusion: The application of continuous midwifery care to Mrs. D is expected to be a benchmark or guideline for midwifery services | en_US |
