Abstract

Background: The decline in MMR can be realized by the availability of guarantees for maternal access to quality health services for pregnant women, delivery assistance by trained health workers in health care facilities, post-natal care for mothers and babies, special care and referrals in case of complications, and health services. postnatal family planning. One of the efforts to reduce maternal, infant and child mortality is to implement Continuity of Care. Objective: To provide continuous midwifery care to pregnant women starting from pregnancy, childbirth, postpartum, newborns and family planning services. Methods: Case studies, documentation with SOAP management and midwifery care using the Varney method. Result : Mrs. R, 31 years old, GIIIPIIA0, the first day of the last menstruation is 16-06-2021, the estimated delivery is 23-03-2022, given 3 visits in the third trimester, experienced cramps in the lower abdomen during pregnancy aged 32-34 weeks, but it can be overcome by educating the mother about the physiological conditions in the third trimester of pregnancy and how to deal with her complaints. R did not experience perineal rupture, the baby boy was born spontaneously, weighed 3300 g, length 49 cm, Apgar score 9/10, the lactation process went smoothly and Mrs. R wants to be an acceptor of the 3-month injection method as a pregnancy control tool. Conclusion: The midwifery care provided, starting from pregnancy to becoming an acceptor for the family planning program, has met the standards of care and the authority of the midwife
