Abstract

Background: Midwifery care that prioritizes continuity of care is very important given to women to get maximum service from a professional or from a small team of professionals.Through the continuity care, clients will be more open about their complaints, feel calm with the assistance they receive during the examination, monitoring the condition of the client and fetus, and additional knowledge, so that all risks that could potentially harm the mother and fetus can be addressed immediately. Purpose : To provide midwifery care to Mrs. J, 25, continuity of care starting from pregnancy, childbirth, postpartum, newborns and becoming an acceptor of family planning programs is carried out to meet the standards of midwifery care and management. Methods: Continuous midwifery care with SOAP documentation in accordance with midwifery care standards. Result : Through continuous midwifery care, it is known that Mrs. J, 25, GIIIII A0 had mild anemia but did not cause complications in pregnancy, childbirth, and the puerperium. The process of giving birth to a baby girl took place spontaneously with a weight of 3,800 grams, body length 49 cm, head circumference 35cm, chest circumference 34cm, W/S: 8/10. Newborn care is given as needed, newborn care takes place normally and without any signs of danger or complications for the baby. At the last visit during the puerperium, the mother was informed about contraception and she decided to become an acceptor of the injectable family planning program. Conclusion : Mrs. J, 25, has mild anemia and is recommended to consume nutritious food, Fe tablets and become an acceptor for a family planning program
