Abstract

Background: MMR is the ratio of maternal mortality during pregnancy, childbirth and the puerperium caused by complications of pregnancy, childbirth, and postpartum or its management but not due to other causes such as accidents or falls in every 100,000 live births, and is also used as an indicator for assess the success of maternal health efforts. In addition to assessing the success of health programs for mothers, it can also be used to assess the health status of the community, because it is very sensitive to the improvement of health services, both in terms of accessibility and quality. Purpose : To provide a midwifery care to Mrs. A, 20, with continuity of care starting from pregnancy, childbirth, postpartum, newborn care, to becoming an acceptor of family planning programs that are provided in accordance with midwifery care and management standards. Methods : This research is a case study documented with SOAP management. Result : Mrs. A. , 20, Gravida 1, Parity 0, Abortion 0, pregnancy in the third trimester; first contact at 37-38 weeks of gestation; at the time of delivery a grade 2 episiotomy was performed and no problems were found in the baby, the newborn care was carried out normally; monitoring is carried out during the postpartum period to assess the normality of the process experienced by the mother until she becomes an acceptor of the family planning program; the mother chooses the 3-month injection, a short-term method, as a means of pregnancy control; and monitoring of the baby is carried out from birth until the end of the neonatal visit to see any possible problems. Conclusion : The care provided, starting from pregnancy to becoming a family planning acceptor, is in accordance with the standards of care and the authority of the midwife.
