dc.description.abstractBackground: Because mothers and children are vulnerable group, the health of mothers and children needs to be given priority in carrying out health efforts and also getting attention from families and the surrounding community. Therefore, an assessment of the health status of mothers and children and the successful of the efforts to improve maternal and child health is important to do. Objective: To improve the health status of mothers and children through midwifery care with continuity of care starting from pregnancy, childbirth, postpartum, newborns, and family planning services. Method : This midwifery care is carried out on an ongoing basis and documented with SOAP management. Result : Mrs. T, 30 years old, GIIIPI1A0, the first day of the last menstrual was 15-06-2021, received 7 times ANC, was constipated at 28-30 weeks of pregnancy but could be overcome by suggesting the mother to consume foods rich in fiber such as green vegetables and fruit ( papaya, banana, etc.); Mrs. T gave birth normally, had a grade II perineal rupture, sutured and no problems were found in the perineal wound care; baby boy was born spontaneously, weight 3000 gr, length 50 cm, head circumference 33 cm, chest circumference 32 cm, Apgar score 7/10; the baby is given breast milk and the umbilical cord is fed on day 5; during the postpartum care, Mrs. T did not experience any complaints, the lactation process went smoothly and the baby wanted to drink breast milk; during the care of the newborn no complications were found; and Mrs. T chose to be an acceptor of the 3-month injection method as a means of pregnancy control. Conclusion : Implementation of midwifery care with continuity of care to Mrs. T is expected to be a benchmark or guideline in midwifery services.en_US
