Abstract: Background : Reproductive health is supported by healthy physical, mental and social conditions, not only free from disease or disability related to reproductive systems, functions, and processes in men and women. Midwives, as one of the oldest professions in the world, have a very important and strategic role in reducing MMR and IMR and preparing the nation's next generation of quality. This can be realized through quality and sustainable midwifery services. Objective : to improve the health status of mothers and children through the application of continuity of care for pregnant women, maternity, postpartum, newborns, and family planning services, carried out in accordance with midwifery care and management standards. Methods : Continuous midwifery care and documentation using SOAP management. Results : During antenatal care, the mother complained of back pain, a physiological complaint in pregnancy, but it can be handled. At the time of delivery, the mother had a second-degree laceration, and was treated with a straight suturing technique. The baby boy was born spontaneously, the Apgar score at minute 1 was 8/10 and at minute 5 it was 10/10, weight 3,300 grams, length 48 cm, head circumference 34cm, chest circumference 35cm, no congenital abnormalities were found. During postpartum care, mothers follow the culture of warming up using charcoal during postpartum recovery, the problem can be resolved and mothers are encouraged to drink more to meet fluid intake to prevent dehydration due to warming up with charcoal. In newborn care, on the 7th day a rash was found in the baby due to the use of diapers every night, but the problem was resolved with ointment and discontinuation of the use of diapers. At the end of the postpartum visit, the mother chose to be a 3-month injection acceptor as a means of pregnancy control. Conclusion : The midwifery care provided, starting from pregnancy to becoming an acceptor of family planning programs, has met the standards of midwifery care and the authority of a midwife.
