dc.description.abstractBackground: A midwifery care should prioritize continuity of care provided to a mother during pregnancy, childbirth, postpartum, newborn and family planning services. Such services are obtained from a professional or from a small team of professionals. Through continuous care, the client will be more open about his complaints, feel calm because he is accompanied during the examination, the condition of the client and the fetus is monitored, and given additional knowledge to prevent complications that threaten the life of the mother and baby. Purpose : To provide a midwifery care to Mrs. C, 19, with continuity of care, starting from pregnancy, childbirth, postpartum, newborn and family planning services following midwifery care and management standards. Methods: Continuous midwifery care with SOAP documentation meets the standards of midwifery care. Result : During continuous midwifery care, it was found that Mrs. C, 19, GIP0A0, experiencing sleep disturbances in the third trimester of pregnancy; no complications were found during pregnancy, the baby girl delivery process was normal and spontaneous, with a weight of 3100 grams, body length 49cm, head circumference 33cm, chest circumference 34 cm, and A/S: 8/10; in the second stage of labor, Mrs. C suffered a grade III rupture but was immediately given grade III hecting with the basting technique; the puerperium is normal without complications; newborn care is provided as needed; no signs of danger or complications were found in the baby; and at the last postpartum visit, the mother was informed about the use of contraception and decided to become an acceptor of the family planning program through the injection method. Conclusion : Midwifery care for Mrs. C, 19, primiparous, with third degree perineal rupture, performed according to the standard of care and authority of the midwife.en_US
