Abstract

Background: Basically pregnancy is a natural or physiological process but under certain conditions it can turn into pathology. In the third trimester of pregnancy, the mother experiences many discomforts such as low back pain and shortness of breath caused by the enlarged fetus pressing on the diaphragm, causing impaired diaphragm function in breathing. Purpose: To provide midwifery care for Mrs.J, 32 years of continuity of care starting from pregnancy, childbirth, newborns, postpartum and family planning which is provided in accordance with midwifery care standards. Method : midwifery continuity of care with SOAP format documentation in accordance with midwifery care standards. Results: Midwifery care for Mrs. J, 32, G4P3A0, gestational age between 38-40 weeks, First Day of Last Menstruation is 10-07-2022, Estimated Date of Delivery is 17-04-2023. Through ANC it is known that the mother has shortness of breath and back pain and can be treated. INC: baby boy born spontaneously at 10.40, crying immediately, given EIB, given an injection of oxytocin 10 UI and the placenta was born spontaneously at 10.45, weight 4300 grams, length 55 cm, Apgar score 7/10, baby was born in healthy condition , was given vitamin K, HB-0, the umbilical cord was broken on the 5th day, postpartum lasted 40 days without any problems, the lactation process went smoothly and Mrs. J uses condoms as a means of birth control. Conclusion: Mrs. J, 32, G4P3A0, it is recommended to maintain personal hygiene, avoid strenuous activities, and get enough rest. Mothers become acceptors for family planning programs and are expected to consult with health workers.
