Abstract

Background: Anternatal care is an effort to prevent the causes of morbidity and mortality in pregnant women and children. The purpose of anternatal care is to prepare the mother physically and mentally as well as possible and save the mother and child during pregnancy, childbirth and the postpartum period so that the mother and child are physically and mentally healthy. Purpose: Implementing midwifery care in the form of continuity of care for Mrs. L, 31, which was carried out in accordance with midwifery care and management standards. Method: Continuity of Care midwifery care and documentation in SOAP format. Result : Mrs. L, 31, GIIIPIIA0, third trimester of pregnancy. The first box is between 32-34 weeks of gestation. The delivery process went smoothly and well, the baby girl was born spontaneously, cried immediately, weighed 3,250 grams, length 48 cm, Head Circumference 31 cm, Chest Circumference 30 cm, Upper Arm Circumference 10 cm, APGAR score 9/10, and the baby was in good condition fit. Postpartum monitoring is carried out to assess the process experienced by the mother until she becomes a program acceptor. Mothers choose the 3-month injection method as a means of controlling pregnancy. Baby monitoring from birth until the end of the neonatal care period has been carried out. Conclusion: Midwifery care provided, starting from pregnancy until the mother becomes an acceptor of the family planning program, has been carried out in accordance with midwifery care standards and midwives' authority
