Abstract: Background: Maternal and child health has become a priority in health care programs. The prevalence of pregnant women with anemia reaches 35 - 75% and the number is increasing with increasing gestational age. Anemia status in the third trimester of pregnancy reached 72.7%, Objective: To provide midwifery continuity of care to Mrs.M in midwifery care starting from pregnancy, childbirth, postpartum, newborn services and family planning which is carried out in accordance with midwifery care standards. Method : Midwifery care is carried out on an ongoing basis through interviews, primary and secondary data collection and documented in SOAP format. Results: Mrs.M, 24, GIP0A0, First Day of Last Menstruation is 20 July 2022, Estimated Date of Delivery is 27 April 2023, in third trimester of pregnancy, Hb level is 10.6 gr/dl and given 30 iron tablets, eats food high in iron. The delivery took place on May 4, 2023, at 03.40, EIB was given and the baby was successfully carried out in the 30th minute, grade II perineal rupture, colostrum was given to the baby, anesthesia and suturing. In the postpartum period, breast milk was immediately given to the baby, the lactation process was going smoothly, there were no signs of infection in the perineal wound and it was dry on the 14th day, the height of the uterine fundus was normal, the lochia did not smell. Baby girl born spontaneously, crying loudly, moving actively, Apgar score 9/10, weight 3400 gr, length 50 cm, given Neo K injection and HB-0 immunization, given umbilical cord care and released on the fifth day. They were given family planning counseling, various contraceptive methods, the mother had not menstruated, the couple planned to choose the implant method as a means of controlling pregnancy. Conclusion: Midwifery care given to Mrs.M, from the third trimester of pregnancy until she became a family planning program acceptor, was carried out in accordance with midwifery care standards
