Abstract: Background: Efforts to reduce MMR can be carried out with the guarantee that every pregnant woman is able to access quality maternal health services during pregnancy, childbirth, postpartum, newborn services, and family planning programs. In the third trimester of pregnancy, mothers often feel uncomfortable with their pregnancy, such as cramps in the lower abdomen. Midwifery care given to Mrs. J, starting from pregnancy, childbirth, postpartum, newborns, to family planning program services carried out in midwifery Continuity of Care. Objective: To provide continuous midwifery care starting from pregnancy, childbirth, postpartum, newborn and family planning program services in the form of midwifery Continuity of Care. Method : This research is a case study and documented in SOAP format. Result : Mrs. J, 25, GIP0A0, at the third trimester visit, between 32-34 weeks of pregnancy, the mother experienced pain in the lower abdomen but it can be managed properly by educating the mother about the physiological conditions in the third trimester of pregnancy and how to deal with the pain. The first stage of labor lasts for 5 hours, during the second stage of labor, a perineal rupture occurs, the birth process takes place normally. Baby boy born spontaneously, weight 3300 gr, length 50 cm, Apgar score 9/10. The lactation process took place smoothly and Mrs. J wanted to become an acceptor for the family planning program by choosing the 3-month injection method. Conclusion: Midwifery care given to the mother, starting from pregnancy until she becomes an acceptor of the family planning program, has been carried out in the form of midwifery Continuity of Care in accordance with the standards of care and authority of midwives
