Abstract: Background: The success of the maternal health program can be measured through the maternal mortality rate (MMR) as an indicator. The maternal mortality rate is calculated based on all maternal deaths during pregnancy, childbirth and childbirth caused by its management, not due to other causes such as accidents. The results of the 2015 Population Survey (SUPAS) showed that the maternal mortality rate was three times higher than in 2015. Perineal rupture is one of the causes of maternal bleeding during the postpartum period. The incidence of perineal rupture in women giving birth in the world in 2020 reached 2.7 million cases, and is predicted to reach 6.3 million in 2050. On the Asian Continent alone 50% of birth mothers experience perineal rupture. Purpose : To provide a midwifery care for Mrs. R with the concept of continuity of care which is preferred starting from pregnancy, childbirth, postpartum, care for newborns, and family planning programs. Methods: this research is a continuous midwifery care accompanied by documentation in the SOAP format. Results: Mrs. R, 38 years old, PIII A0, First day of last menstrual period was 23-06-2022, ANC was given 8 times, the delivery process took place normally accompanied by grade II perineal rupture, the wound was sutured and no problems were found in wound care. Baby boy born spontaneously, weight 3900 gr, length 51.5 cm, head circumference 35.5 cm, chest circumference 33 cm. Monitoring during the postpartum period is carried out to assess the normalcy of the process experienced by the mother until she becomes an acceptor of the family planning program. Mrs.R chose the implant method as a pregnancy control tool. Conclusion: The care provided starting from pregnancy until the mother becomes the acceptor of the family planning program has been carried out in accordance with the standards of care and authority of midwives
