Abstract: Background: The Maternal Mortality Rate (MMR) is an indicator of the success of maternal health efforts. To realize maternal health efforts, continuous midwifery care in the form of continuity of care, starting from pregnancy, childbirth, postpartum, newborns, and family planning needs to be carried out. Objective: To provide continuous midwifery care in the form of continuity of care starting from pregnancy, childbirth, postpartum, newborns, until the mother becomes a family planning program acceptor and is carried out in accordance with midwifery care standards. Method: Case study with a continuous midwifery care approach and documentation in SOAP format. Results: Mrs.A, 22 years old, G1P0A0, Hb 18 gr/dL, experienced low back pain during 30-31 weeks of pregnancy. The delivery process took place normally without any problems during labour. Baby girl born spontaneously, weight 3300 g, length 47 cm, A/S 9/10. Babies get EIB. The postpartum period proceeded normally, the lactation process went smoothly and Mrs. A chose to become a family planning acceptor and chose the 3-month injection method as a pregnancy control tool. Conclusion: Midwifery care provided, starting from pregnancy until the mother becomes an acceptor for the family planning program, has been carried out in accordance with the standards of care and authority of midwives
