dc.description.abstract | Background: The Maternal Mortality Rate in 2021 in Indonesia is 6,856 people per 100,000 live births, while in North Sumatra it is 248 per 100,000 live births. Reducing MMR as an improvement in the quality of life of mothers has become a world health priority. One of the steps recommended by WHO is midwifery care in the form of continuity of care (CoC). Objective: To provide continuing midwifery care for mothers, from pregnancy, childbirth, postpartum, newborn and family planning using a midwifery management approach. Method : Continuity of Care midwifery care with midwifery management documented in SOAP format. Result : Mrs. R, 28 years Gravida III, Paritas II, Abortion 0. The first contact was at 28-29 weeks of pregnancy, ANC visits were made 3 times, the complaints experienced by the mother during pregnancy were physiological changes. The first stage of labor lasted 6 hours, if the second stage of perineal laceration occurred, hecting was performed using a continuous suture technique. The postpartum period lasts 42 days and proceeds normally. The baby was born with severe asphyxia (apgar score 2/3), resuscitation and newborn care were carried out. On the 6th day there was degree I physiological jaundice, care was taken by drying the baby for 15 minutes in the morning, after the 10th day the jaundice was resolved. The mother chose the IUD as a means of contraception. Conclusion: Continuous Midwifery Care in the form of continuity of care increases the knowledge and degree of health of mothers and babies. | en_US |
