Abstract: | Background: The Maternal Mortality Rate (MMR) in 2022 is 207 per 100,000 live births, the target of 183 per 100,000 live births has not been achieved, and in 2024 it has also not been achieved due to an excess of 70 deaths per 100,000 live births. The high MMR and Infant Mortality Rate require continuous midwifery care in the form of Continuity of Care (CoC). Objective: Able to provide continuous midwifery care (Continuity of Care) to pregnant women, childbirth, postpartum, newborns and family planning services with a midwifery management approach. Method : Midwifery care in the form of continuity of care using midwifery management and documentation in the SOAP format. Result : Mrs. M, 28 years, Gravida II, Paritas I, Abortion 0. Her first contact was at 28-29 weeks of pregnancy, she had complaints of flat nipples, vaginal discharge and low back pain and most of the complaints were resolved, except for flat nipples which is a congenital anomaly. Pregnancy visits are given 3 times. The delivery lasted for 2.5 hours and the perineal laceration was done second degree. Hecting perineum with continuous suture technique. In newborn care, on the 5th day, grade I jaundice was found and the baby was dried in the sun for 15 minutes and on the 9th day it was resolved. The postpartum period normally lasts 42 days and you choose an IUD. Conclusion: Continuity of Care provides increased knowledge and degree of maternal and neonatal health.
