Abstract

Background: Pregnant women are classified as suffering from anemia if their Hb levels in the first and third trimesters are <11 g/dl, and in the second trimester they are <10.5 g/dl. The incidence of perineal rupture in the world occurs in 2.7 million birth mothers. This figure is estimated to reach 6.3 million in 2020. Perineal lacerations in Indonesia are experienced by 75% of women giving birth vaginally. Purpose: Provide continuous midwifery care in the form of continuity of care to Mrs. F on pregnancy, postpartum, newborn, family planning and to Mrs. E during childbirth. Methods: a continuing midwifery care and documentation in SOAP format and researching secondary data. Result: Mrs. F 35 years old G5P4A0, First day of last menstrual period was 15 June 2022, Hb level in third trimester was 9.5 gr/dl and given 40 Fe tablets, on visit II, Hb level was 12.5 gr/dl, HIV test was carried out. During labor, perineal rupture was done second degree, suturing with basting technique, IMD was successfully done in the 35th minute, colostrum was given to the baby. During the postpartum period, the lactation process proceeds smoothly, the height of the uterine fundus is normal, the baby feeds voraciously. A baby boy was born spontaneously on 01 April 2023 at 14.15 WIB, weight 3300 grams, length 50 cm, APGAR score 9/10, given Vit K 0.5 cc & HB 0 immunization with a dose of 0.5 cc, umbilical cord ruptured on day 7. In family planning care, counseling is given about tubectomy, the mother has not menstruated, and the couple decides to use the tubectomy method. Conclusion: Care from pregnancy to family planning is in accordance with midwifery care standards and the authority of a midwife
