dc.description.abstractBackground: Basically, pregnancy in a woman is a natural or physiological process, but under certain conditions it can turn into pathology and if not handled properly will result in an emergency that can threaten the life of the mother and fetus. Therefore, every pregnant woman needs continuous monitoring to ensure that the condition of the mother and fetus is in good health, which can be done through continuous midwifery care in the form of continuity of care. Objective: Able to provide a continuous midwifery care or midwifery continuity of care since pregnancy, childbirth, postpartum, newborn care until the mother becomes a family planning program acceptor, and is given in accordance with a midwifery management approach. Methods: This research is a case study designed with sustainable Midwifery Care design and documentation in the Subjective, Objective, Assessment and Planning (SOAP) format. Result : Mrs.S, 30, GIP0A0. The first contact during pregnancy is between 31-32 weeks and the pregnancy is normal. Delivery is carried out by sectio caesarea. Baby boy spontaneously crying, weight 3200 gram, length 50 cm, Apgar score 9/10. Newborn care takes place normally. Postpartum care proceeded normally. Mrs.S became the acceptor of the family planning program and chose the 3-month injection method. Conclusion: Care given to the mother, starting from pregnancy until she becomes an acceptor for the family planning program, has been carried out in accordance with midwifery care standards that can improve the health of mothers and babies.en_US
