Abstract

Background: Midwifery care should prioritize continuous midwifery services in the form of continuity of care provided to mothers during pregnancy, childbirth, postpartum, newborns and family planning programs. Continuing midwifery care plays a role in increasing the health status of mothers and children. Purpose: To provide continuous midwifery care in the form of continuity of care to mothers, starting from pregnancy, childbirth, postpartum, newborn and family planning in accordance with midwifery care standards. Methods: this research is a case study of a continuous midwifery care and documented in SOAP format. Result : Mrs. T 22 years, GIP0A0, experienced lower back pain during pregnancy, during the puerperium with the lactation process not smooth, baby boy born spontaneously, A/S: 8/10, weight 3400 gram, length 50 cm, baby born normally . Mrs.T became an injecting acceptor -1-month, Mrs.A's delivery was accompanied by first-degree perineal lacerations and sutured with interrupted suture technique. Conclusion: Midwifery care provided starting from pregnancy until the mother becomes a family planning program acceptor has been carried out in accordance with midwifery care standards
