Abstract: Background: Continuity of care in midwifery care is a series of activities as a whole and continues starting from the period of pregnancy, childbirth, postpartum, newborn and family planning care according to women's health needs. Continuity of care aims to improve overall care through midwifery management, providing information and building relationships. Objective: To provide midwifery care to Mrs. T aged 28 in the format of continuity of care, which is provided starting from pregnancy, childbirth, postpartum, newborn and family planning in accordance with midwifery care standards and midwifery management. Method : A continuous midwifery care and documented in SOAP format. Results: During midwifery care it was found that during pregnancy Mrs. T complained of back pain and it was handled properly. At the time of delivery, a grade I perineal rupture was performed and it was handled properly. The baby boy was born spontaneously and was healthy and normal, no complications were found during the puerperium, all processes took place normally and naturally until the mother became an acceptor for the family planning program, and the mother chose the implant method as a pregnancy control tool. Conclusion: The care provided starting from pregnancy until the mother becomes a family planning acceptor is in accordance with the standards of care and authority of midwives
