Abstract

Background: The Maternal Mortality Rate (MMR) is an indicator to see the success of maternal health efforts. Health services for pregnant women should guarantee protection for pregnant women and the fetus, such as early detection of risk factors, prevention and early treatment of complications in pregnancy. During pregnancy, there will be several changes that will disturb the mother's comfort, such as shortness of breath and low back pain. One of the factors that affect the lactation process is the flat shape of the nipples, which can inhibit the lactation process Objective: To provide midwifery care in the form of Continuity of Care starting from pregnancy, childbirth, postpartum, newborn and family planning services. Method: a case study of continuous midwifery care and documentation in SOAP format. Result: Mrs.Y, 24, GIP0A0, third trimester of pregnancy, complaints are flat nipples, shortness of breath and low back pain. Shortness of breath and low back pain can be resolved except for flat nipples due to congenital abnormalities. Care for the first stage of labor proceeded normally, in the second stage, a second degree perineal laceration was performed with a continuous suturing technique. The baby was born normally, weighing 3700 grams. The postpartum period proceeds normally. On the 24th day the baby stops drinking milk because milk production has stopped. Mrs.Y used the technique of interrupted intercourse for pregnancy control. Conclusion: Continuity of Care can increase knowledge and degree of maternal and child health
