Abstract

Background: The success of a country's health services is determined by the ratio of maternal mortality and perinatal mortality rates. Maternal mortality is caused during pregnancy, childbirth and puerperineum. One of the efforts to reduce maternal, infant and child mortalitr is implementing continuous care (continuity of care). Purpose: To provide a midwifery care given to Mrs. N, 30, in the form of continuity of care starting from pregnancy, and midwifery care for Mrs. R, 24, starting from childbirth, postpartum, newborn to family planning services carried out in accordance with midwifery care and management standards. Method : Continuing midwifery care and documentation in SOAP format, carried out in accordance with midwifery care standards. Results: Midwifery care Mrs. N, 30, G3 P2 A0, First Day of Last Menstruation is 09-06-2022, Estimated Date of Delivery is 16-03-2023, given 2 visits during pregnancy. Continuous midwifery care for Mrs. R, 24, GI P0 A0. Baby girl born spontaneously, weight 2500 grams, length 47 cm, head circumference is 33 cm, chest circumference is 33 cm, Apgar 9/10, with female gender. Delivery to Mrs. R had a grade II rupture and hecting a basting technique. In BBL care there were no problems found. During the postpartum visit, they were informed that there were no signs of danger. Conclusion: Mrs. R, primipara, 24, gave birth with second degree perineal rupture.
