Abstract: Background: Midwifery care in the form of Continuity of care (COC) is a midwifery care on an ongoing basis starting from pregnancy to family planning program services, carried out as an effort to reduce maternal and infant mortality. Objective: To provide continuous midwifery care starting from pregnancy, childbirth, postpartum, newborn and family planning services. Methods: This research is a case study and documented the Varney method and SOAP format. Results: through midwifery care provided to Mrs. M, 30, GIIPIA0, it is known that the First Day of the Last Menstruation is 20-06-2022, the Estimated Date of Delivery is 27-03-2023. Experiencing lower abdominal pain when the pregnancy is 31-32 weeks old but can be overcome by educating the mother how to deal with the complaints she feels. The birth process took place normally, Mrs. M had grade 1 perineal rupture. Baby girl was born spontaneously, weight 3300 gr, length 48 cm, Apgar score 9/10. The lactation process took place smoothly and Mrs. M chose the implant method as a pregnancy control tool. Conclusion: The midwifery care provided, starting from pregnancy until the mother becomes the acceptor of the family planning program, is in accordance with the standards of care and authority of midwives
