Abstract

Background: Antenatal Care (ANC) is an effort to maintain the health of pregnant women and their babies. To achieve this, it is necessary to carry out continuous care in the form of continuity of care starting from pregnancy, childbirth, postpartum, newborn and family planning care. Objective: To provide continuous midwifery care in the form of Continuity of Care for mothers from pregnancy, childbirth, postpartum, newborn until the mother gets family planning services. Method: A case study with a continuous midwifery care approach and documentation in SOAP format. Results: Mrs.Y, 26 years old, G2P1A0, suffere from hemorrhoids at 28-30 weeks' gestation. The delivery process proceeded normally and a second degree perineal laceration was performed with 7 stitches and the perineal wound dried on the seventh day. Baby girl delivered spontaneously, weight 3600 g, length 49 cm, A/S 9/10. Baby gets IEB. The postpartum period proceeded normally, the lactation process proceeded smoothly and Mrs. Y became the acceptor for the 3-month injection. Conclusion: Midwifery care provided, starting from pregnancy until the mother becomes a family planning program acceptor, has been carried out in accordance with the standards of care and authority of midwives.
